2. Treat one patient (local only) for stop with laser to quit smoking or laser weight control ($400 per additional patient).
3. Train you to set up and use the laser, use it for laser stop smoking and laser weight control and will provide you with various treatment protocols.
4. KickButts LLC will set up a mutually acceptable appointment time to train you using Skype via a web cam (download from www.skype.com). Client's web cam must be operational 24 hours prior to the appointment time.
Client must agree to $100 cancellation fee if the appointment with KickButts, LLC is canceled with less than 24 hours notice.
